Dr. Chatburn provides compassionate, personalized, in-home end-of-life care directly to patients and their communities, ensuring comfort, dignity, and support in the intimacy and comfort of your own home.
She encourages patients and families to seek the care of a traditional hospice through Medicare or other similar insurance benefits. For desired interventions and visits that aren’t covered by the interdisciplinary hospice team at the time desired, Dr. Chatburn can offer an additional layer of support as an outside consultant.
Chatburn Companioning offers a range of telehealth and in-person services, including prognostic evaluation, clinical consultation, symptom management, goals of care conference facilitation, ethics conflict mediation, and grief integration for patients and their intimate communities related to terminal or life-threatening illness. Additionally, we offer in-home physician accompaniment for patients and loved ones during the last hours of life.
